GDPR = personal data policy
for Copenhagen Mobile Veterinarian

1. Introduction. 1.1 The basic necessary collection of knowledge. 1.1.1. Copenhagen Mobile Veterinarian will electronically and otherwise systematically process personal data in connection with your registration as a customer. Copenhagen Mobile Veterinarian is obliged to be able to inform you about what information is collected, registered, disclosed or processed. More on this below. 1.1.2. The overall purpose of data collection is the administration and the knowledge needed associated with consultations and treatment for your animal(s), in connection with the conclusion of the agreement, the start, progress and completion of the treatment process, including in relation to the services etc. to which you are entitled, as well as the obligations to which you as an animal owner are subject. This is a necessary measure in order to be able to effectively treat your animal(s). 1.2 Data responsibility for contract information 1.2.1. Copenhagen Mobile Veterinarian is the data controller for information that is processed and is responsible, according to data protection legislation, for your personal data to be stored in accordance with the provisions of the law. 1.2.2. All personal data mentioned above must be processed in accordance with the guidelines and framework set by data protection legislation and any other legislation. 1.2.3. You can contact Copenhagen Mobile Veterinarian at any time by email at cfunderg@gmail.com if you have any questions about the handling of your personal data, or if you have any questions or rights to assert according to data protection legislation. 2. Processing of personal data 2.1 Customer file 2.1.1. Upon contact and agreement with me, I will create a record with information collected about you and your animal(s) in my electronic customer file. All personal data generally originates only from you. 2.1.2. Under your electronic medical record card, I register as standard the identification number of your animal(s) and owner's contact information, owner's name, private address, private and work telephone number, and email address. In addition, medical record information about you or your animals is registered on an ongoing basis. Finally, information about other special circumstances is registered, such as insurance, agreements on payment and possible credit (which I generally never give), health plans and agreements on other payers. 2.1.3. The information in the system can be combined with information about other customers, for example to create various statistics and statements for internal use, as well as disclosure upon request from public authorities. 2.1.4. The legal basis for data storage in connection with the customer relationship is initially Section 6, Subsection 1, No. 2 of the Personal Data Act (as of 25 May 2018, Article 6 (1), Letter b) of the General Data Protection Regulation). 2.2 X-ray and scan images 2.2.1. If X-ray and/or scan images are taken, I will register your name and identification information about your animal(s). 2.2.2. The legal basis for the processing is basically the Danish Personal Data Act, Section 6, Subsection 1, No. 2 (as of 25 May 2018, Article 6 (1), letter b) of the General Data Protection Regulation 2.3 Prescribing, reporting and records of prescription-only medicines 2.3.1. Veterinary prescriptions contain your personal data, which is your name and address. It is stipulated by law which information, including personal data, a veterinary prescription must contain in order for a pharmacy to approve the issued veterinary prescription and for you as a customer to be able to receive the prescribed medicine for your animal(s). Veterinary prescriptions are authorised by the Executive Order on Prescriptions and Dose Dispensing of Medicinal Products. 2.3.2. Copenhagen Mobile Veterinary does not offer services as a livestock veterinarian. Should this or similar tasks nevertheless occur, the procedure is that personal data on you and identification information on your animals are reported to the Veterinary Register VetReg in the Danish Veterinary and Food Administration. It is authorised by law* that health advice agreements for cattle, pigs and mink that have been concluded between you and Copenhagen Mobile Veterinarian must be reported to the Veterinary Register VetReg in the Danish Veterinary and Food Administration. Copenhagen Mobile Veterinarian only offers this type of veterinary treatment if there is a health or political emergency. 2.3.3. The legal basis is initially Section 6, subsection 1, no. 3 of the Personal Data Act (as of 25 May 2018, Article 6 (1), letter c) of the General Data Protection Regulation). * The legal basis is the Executive Order on Health Advisory Agreements for Cattle Herds, the Executive Order on Health Advisory Agreements for Pig Herds and the Executive Order on Mandatory Health Advisory in Mink Farms. 2.4 E-mails 2.4.1 I centrally log e-mails that are sent from the customer to Copenhagen Mobile Veterinarian and sent from Copenhagen Mobile Veterinarian to the customer. This means that I register who the email is from, who it was sent to or from, when the email was sent and received. The content of the emails that are received and sent to Copenhagen Mobile Veterinarian will be recorded in the customer's or the customer's animal's electronic journal. 2.4.2. As a customer of Copenhagen Mobile Veterinarian, you will – if you consent in advance – be able to receive reminders of treatments that have been agreed upon or treatments that should be repeated within a set date, such as revaccination. 2.4.3. The legal basis is basically Section 6, Paragraph 1, No. 2 of the Personal Data Act (as of 25 May 2018, Article 6 (1), Letter b of the General Data Protection Regulation). 2.5 Calendar system 2.5.1. In our electronic calendar, I register the agreements I have with the customer. In this connection, I state your name and, if applicable, identification information about you or your animals. 2.5.2. The legal basis is basically Section 6, Paragraph 1, No. 2 of the Personal Data Act (as of 25 May 2018, Article 6 (1), Letter b of the General Data Protection Regulation). 2.6 Laboratory tests and results 2.6.1. If samples are taken from your animal(s), if biochemistry or other tests are performed on blood and parasite tests on feces, Copenhagen Mobile Veterinarian will be able to culture/analyze the results of some of the samples itself. If Copenhagen Mobile Veterinarian does not have the equipment to analyze the test results of a sample taken, the sample will be sent to an external laboratory in Denmark, EU/EEA countries and possibly countries outside the EU/EEA. When the sample is sent and the results are received, this will contain personal information about you, such as your name and the identification number of your animal(s). Your personal data will be processed, cf. section 3.2 of this letter. 2.6.2. The legal basis is initially section 6(1)(2) of the Danish Personal Data Act (as of 25 May 2018, Article 6(1)(b) of the General Data Protection Regulation). 2.7. Registration of animals in registers 2.7.1. Registration of dogs in the Danish Dog Register. You have, cf. the Executive Order on Dogs (the Dog Act), an obligation to have your dog registered in the Danish Dog Register and to re-register your dog if you sell it, just as you must notify the Danish Dog Register of the death of your dog. If you enter into an agreement with Copenhagen Mobile Veterinarian that I register your dog in the Danish Dog Register on your behalf, I will send personal information about you and identification information about your dog to the Danish Dog Register. If Copenhagen Mobile Veterinarian registers your dog for you, Copenhagen Mobile Veterinarian will charge the registration fee on behalf of the Danish Dog Register. 2.7.2. Registration of cats. It is not mandatory in Denmark to have your cat registered, but it is a good idea, as an unregistered cat is considered stray. If you want your cat registered in the Danish Cat Register, and you enter into an agreement with Copenhagen Mobile Veterinarian that I register your cat in the Danish Cat Register on your behalf, I will send your personal information and identification information about your cat to the Danish Cat Register. If Copenhagen Mobile Veterinarian registers your cat for you, Copenhagen Mobile Veterinarian will charge the registration fee on behalf of the Danish Cat Register. 2.7.3. Registration of horses in SEGES All horses in Denmark must have a passport with medicine pages, cf. the Executive Order on the Identification of Animals of the Equine Family. The Executive Order supplements EU Regulation 504/2008 of 6 June 2008. It is SEGES that issues horse passports in Denmark and thus registers horses. It is also mandatory by law to have your horse change ownership, and to register departures (dead horses) to SEGES. If Copenhagen Mobile Veterinarian on your behalf handles or assists you with the above, I will use and send personal data about you and identification information about your horse to relevant persons, private companies and authorities. 2.7.4. Registration of cattle, pigs, sheep and goats in the Central Livestock Register (CHR) All herds of cattle, pigs, sheep and goats must be registered in the CHR, cf. the executive order on registration of herds in the CHR. Cattle, pigs, sheep and goats must be marked with specially approved ear tags. At the same time, the properties on which the animals are located must be registered in the CHR. The animal owner/farmer himself creates and deregisters his herd on landbrugsindberetning.dk. If Copenhagen Mobile Veterinarian on your behalf assists you with the above, I will use and possibly send personal data about you and identification information about your animals to relevant persons, private companies and authorities. The registration fee will be passed on to the end customer. 2.7.5. Legal basis The legal basis is basically the Personal Data Act, Section 6, Paragraph 1, No. 2 (as of May 25, 2018, Article 6 (1), Letter b) of the General Data Protection Regulation). 2.8 Payment 2.8.1. Copenhagen Mobile Veterinarian's payment handling system is linked to the customer database, which is why your personal data is processed in accordance with Section 2.1 of this letter. 2.8.2. Credit is generally not granted. However, if a credit agreement is nevertheless concluded between you as a customer and Copenhagen Mobile Veterinarian, this will generally only be possible if you provide your civil registration number. Declaration of Consent 2.8.2. Credit is generally not granted. Storage of personal data 3.1.1. The information about you is stored at the company's address. 3.1.2. Certain information is also stored with our data processors in connection with the operational and IT security tasks (such as external hosting of IT programs, software backup, etc.) that I have solved externally. Storage of the information with external data processors who are partners is subject to Danish data protection regulations and data processing agreements have been entered into with data processors to ensure that your information is not misused or falls into the hands of unauthorized persons. 3.1.3. Your personal data may be entrusted to our lawyer or auditor for use in solving a specific task. This may, for example, be the case if Copenhagen Mobile Veterinarian has used legal or audit assistance in a case in which you are involved, and where the relevant personal data must therefore be used by our auditor or lawyer. 3.2. Transfer to third countries 3.2.1. Some of your data may be transferred to countries located outside the EU/EEA. This is because our data processor may be located in countries outside the EU/EEA. A separate data processing agreement will have been established with this or these data processors. 3.2.2. Transfer of your personal data to countries outside the EU/EEA takes place on the basis of the EU's standard contracts for the transfer of personal data to third countries, which thereby guarantee an adequate level of protection.
